Headaches and Clary Sage Oil: Know Your Scent Triggers
Your head hurts, a bottle of something fragrant is within reach, and it seems like a natural thing to try. Before you open it, it is worth knowing that for some people a strong scent is part of the headache story, not the answer to it. Clary sage oil is a rich, herbaceous aroma that we love, and we sell it as a fragrance and cosmetic ingredient, not as a headache product.

The NHS says to call 999 or go to A&E if a headache comes with a seizure, numbness or weakness in the body or face, a sudden and extremely painful start, a head injury within the last 3 months, trouble speaking, balancing, walking or remembering, drowsiness or confusion, loss of vision, a rash that does not fade when a glass is rolled over it, or a very high temperature, a stiff neck, or bright lights bothering you (NHS, Headaches). If any of that sounds like you or your child, call first. No bottle belongs in that moment.
For everyday headaches, the same page lists common causes that have nothing to do with fragrance: a cold or flu, stress, too much alcohol, bad posture, eyesight problems, skipped meals, not drinking enough, taking too many painkillers, and hormone headaches around a period or during menopause. It suggests keeping a headache diary to spot your own triggers. That is the most useful thing on this page, and we would put it ahead of any oil.
What the oil trials were really about
The headache trials people point to used two other oils, and the details matter.
A German study of peppermint oil followed 41 people with tension-type headache through 164 attacks. The preparation was 10 g of peppermint oil made up to 100 with ethanol (a 10% solution), spread across the forehead and temples and repeated after 15 and 30 minutes (Göbel et al., 1996). A separate Iranian trial had 47 people with migraine inhale lavender oil for 15 minutes, with liquid paraffin as the comparison; the lavender group reported a larger fall in headache severity (3.6 points against 1.6 on a visual scale) (Sasannejad et al., 2012).
Neither used clary sage, and neither tells us what clary sage would do. Peppermint at 10% is also five times our highest skin dilution of 2%, so it is not a recipe to borrow. We read these studies as a reminder that "essential oils and headaches" is a topic with different oils, different doses and different headache types, and that you cannot swap one oil in for another and expect the same result.
When a scent is the trigger
This is the finding we would want a friend to know. In a Brazilian study of 158 people with primary headaches, 72 of whom had migraine, exposure to an odour set off a headache in 25 of the 72 people with migraine (34.7%) and in none of the 86 people with other primary headaches (Silva-Néto et al., 2017). The odours were not clary sage, so this says nothing specific about our oil. It does say that if you live with migraine, a strong aroma held near your face is a reasonable thing to approach with care.
Our practical reading: if a smell has ever started a headache for you, keep aromatic oils out of your headache days and mention it to your doctor. Clary sage is a full-bodied, lingering scent, so a little goes a long way even for people without that history.
If you simply love the scent
Plenty of people do, and a headache-free day is the right time to enjoy it. Our oil is sold undiluted, so we recommend diluting to 0.5% to 2% in a carrier oil from a craft shop or grocery before it touches skin, patch testing first, and keeping it well away from your eyes. Temples sit close to the eyes, which is a good reason to choose the lightest dilution and to skip that spot altogether if you are unsure. Our dilution guide and patch test guide walk through the steps.
A few people should skip clary sage altogether. We avoid it in pregnancy, and we say it is not the oil to reach for before driving. Children and pets should stay clear of it. If you notice a skin reaction, our side effects article covers what to do. For the wider picture of what clary sage research does and does not cover, see our evidence overview and the stress, mood and sleep write-up.
Headaches that keep coming back deserve a conversation with a doctor, and a headache diary will make that conversation shorter and more useful.
